Formal Field Use
Formal field use includes games for one or more teams or practices for multiple teams, with a league, organization, school or company paying for the field. The group has official nonprofit or commercial status, and has its own bank account.

Getting Started Review the 2012 Athletic Field Scheduling Guide , which includes everything about the field scheduling and allocation processes, including billing due dates and field care responsibilities. Key sections include:
Field Scheduling Timeline – Calendar of submission deadlines for requests based on months and sports wanted.
Field Rental Fees – Chart of fees including field rental, lights and other charges.
Lights and Darkness Schedule – Listing to determine the time of dusk for fields without lights or when light fees will be charged based on requested dates.
List of Fields - Complete list of city and school district fields scheduled through the Parks Scheduling Office, including addresses, sizes and season dates.
City and District Projects and Field Closures – Current and upcoming projects and planned closures that impact availability. Subject to change.
Requesting Fields For your convenience, forms can be submitted via email. If a signature is required, request a Client ID# to send forms via email or submit via scanned email, fax, mail or drop-off with a signature.
Request for Formal Field Use - Official form to provide the Parks Scheduling Office with the organization's contact information and details about field time being requested.
Certificate of Insurance – Submit a current Certificate of Insurance meeting our updated requirements: The City of Bellevue must be named as additional insured by listing the City of Bellevue, P.O. Box 90012, Bellevue, WA 98009-9012 as certificate holder. Minimum amount of general liability is $1,000,000 per occurrence, $2,000,000 aggregate.
List of Board Members (if applicable) – Including names, addresses, phone numbers and email addresses.
Proof of Non-Profit Status (if applicable) – Documentation from IRS or Washington State.
Credit Card Authorization Form (if applicable) – to authorize the City of Bellevue to keep the account number on file for future billings.
Before and During the Season:
Update Form for Field Use - To update current reservations. Updates include change in the date or location; adjustments in the timeslot; return of future reservations; credit for past reservations due to weather, field conditions or lights; game prep requests, change to pitching distance, etc.
Request Form for Additional Field Use - To request use in addition to field time already confirmed, including simple requests for additional date, timeslot or field(s).
Concussion Compliance Form - To confirm your organization's compliance to the Zackery Lystedt Law (HB-1824 - Head Injury).
Field Payments – Payment for the first month of use is due at the time of the initial confirmation. Payment of additional monthly field fees are due by the 15th of the month prior. For some ongoing groups, a new payment plan is being offered that would mean an upfront payment and then payments due the month after use. If interested, please contact the Parks Scheduling Office for more information.
After the Season: Within one month after the season has finished, please submit these forms:
